If you have recently been diagnosed with Polycystic Ovarian Syndrome (PCOS), or suspect you might have it, you have likely spent hours trawling through online forums. You have probably read that you must cut out every carbohydrate, lose weight, or take generic blood-sugar medications just to get your cycle back.


But then you look at your own body and feel completely confused. Perhaps you don’t struggle with your weight at all, your diet is already pristine, or your ultrasound didn’t actually show any classic "cysts" on your ovaries.

The Great Misnomer: Ovarian Cysts


Before diving into the types, let’s clear up a significant piece of misinformation. The "cysts" described in PCOS are not actually abnormal cysts at all. They are simply tiny, underdeveloped egg follicles. Because of a hormonal communication breakdown, these follicles never quite get the signal to mature and release an egg (ovulate). Instead, they stall in the growth phase, clustering together like a string of pearls.


You can have PCOS without having these follicles visible on an ultrasound, and you can have the follicles without having the full syndrome. Diagnosis actually relies on a combination of irregular cycles, elevated male hormones (androgens), and ultrasound appearances.

The 4 Types of PCOS: Which One Are You?


To successfully restore your periods, clear your skin, and reclaim your energy, we must identify your specific driver.


1. Insulin-Resistant PCOS (The Most Common Type)


This accounts for roughly 70% of cases. High insulin levels prevent the ovaries from ovulating properly and signal them to produce excess testosterone instead.


Signs: Stubborn weight gain (particularly around the abdomen), intense sugar cravings, fatigue after meals, acanthosis nigricans (darkened patches of skin around the neck or armpits), and adult acne.


The Focus: Improving cellular insulin sensitivity through protein-first nutrition, specific fibres, and regular movement.



2. Inflammatory PCOS


In this type, chronic, systemic inflammation causes the ovaries to overproduce male hormones, blocking regular ovulation.


Signs: Chronic fatigue, unexplained joint pain, headaches, skin conditions like eczema or psoriasis, and recurrent gut issues (like IBS). Your blood tests might show elevated inflammatory markers like high-sensitivity CRP.

The Focus: Identifying and removing inflammatory triggers (such as hidden food intolerances, chronic environmental toxins, or gut dysbiosis) and using natural anti-inflammatory protocols.


3. Adrenal PCOS


While standard PCOS is driven by the ovaries overproducing testosterone, Adrenal PCOS is driven by an overreaction to stress. This type accounts for about 10% of diagnoses and is often found in women who do not fit the typical insulin-resistant profile.


Signs: You have high levels of DHEA-S (an adrenal androgen) on your blood tests, but your standard testosterone and fasting insulin levels are completely optimal. You likely feel chronically stressed, anxious, or suffer from sleep disruptions.

The Focus: Calming the nervous system, supporting the HPA (hypothalamic-pituitary-adrenal) axis, and building stress resilience.


4. Post-Pill PCOS


This type occurs as a temporary rebound effect after stopping oral contraceptive pills. The synthetic hormones in the pill suppress communication between your brain and your ovaries. When you stop, the brain can overcompensate, causing a temporary surge in androgens.


Signs: Your cycles were completely regular before you went on the pill, but now your skin is breaking out, your hair is thinning, and your periods haven't returned
