The Adrenal-Thyroid Pivot: Why You Can’t Fix One Without the Other

For many women, the missing piece of the puzzle isn't just the thyroid—it’s the relationship between the thyroid and the adrenal glands. This is the Adrenal-Thyroid Pivot, and if you try to fix one while ignoring the other, you’re essentially trying to fly a plane with only one wing.



Understanding the Master Controller: The HPA Axis

To understand why your thyroid medication might not be working, we have to look at the HPA Axis (Hypothalamic-Pituitary-Adrenal axis). Think of this as your body’s internal thermostat and alarm system combined.


  1. The Hypothalamus senses stress and signals the Pituitary.

  2. The Pituitary tells the Adrenals to release cortisol (your stress hormone).

  3. The Adrenals respond, but they don't work in a vacuum; they send feedback that tells the thyroid to either speed up or slow down.


When you are under chronic stress—whether from work, lack of sleep, or undereating—your HPA axis stays "on." This creates a domino effect that hits your metabolism hard.

The Cortisol Clog: Why Your T4 Isn't Becoming T3

This is where the biochemistry gets tricky. Your thyroid primarily produces T4 (thyroxine), which is an inactive storage hormone. For you to feel energetic, your body must convert that T4 into T3 (triiodothyronine), the active form that fuels your cells.

High cortisol acts like a roadblock in this conversion process.

When cortisol is chronically high, it does two things:

  • Inhibits Conversion: It suppresses the enzymes responsible for turning T4 into T3.

  • Increases Reverse T3 (rT3): Instead of making active T3, your body starts producing Reverse T3. Think of rT3 as the "brake pedal" for your metabolism. It mimics T3 and occupies the receptors, but it doesn't provide any energy, leaving you feeling exhausted and stuck in fat-storage mode.

Adrenal Fatigue and Weight Gain


The connection between adrenal fatigue (or HPA axis dysfunction) and weight gain is a frustrating cycle. When your adrenals are overworked, your body enters survival mode. High cortisol levels signal the body to store fat—specifically around the midsection—to ensure you have "fuel" for a perceived emergency.

Because your thyroid is being suppressed by this same stress response, your basal metabolic rate drops. You end up eating like a bird and exercising like an athlete, but the scale won't budge because your Adrenal-Thyroid Pivot is skewed toward preservation rather than energy expenditure.

Breaking the Cycle

If you want to reclaim your energy, you have to stop treating the thyroid in isolation.

  • Test, Don't Guess: Ask for a full thyroid panel (including Free T3 and Reverse T3 and a 4-point salivary cortisol test.

  • Support the Adrenals First: If you jump straight into aggressive thyroid medication or intense HIIT workouts without stabilising the HPA axis, you may feel even more "wired" and eventually crash.

  • Prioritise Rest: Sleep is the only time your HPA axis truly resets.
